Emergencies

Appointment Requests

Submitting the form is a request, not a confirmed appointment. Our team will contact you to confirm a time. Please do not include medical details in the message field; the form is not a secure clinical channel.

Telehealth

Telehealth visits are offered where clinically appropriate and where permitted by Colorado law. Telehealth is not suitable for every condition, may require an in-person follow-up, and depends on technology that can fail. Patients must be physically located in Colorado at the time of a visit unless otherwise permitted by law.

Message frequency varies. Message and data rates may apply. Reply STOP at any time to opt out, or HELP for help. Consent is not a condition of purchase or of receiving care. SMS consent and phone numbers are never shared with or sold to third parties for marketing purposes. Carriers are not liable for delayed or undelivered messages.

You agree not to misuse this site — including attempting to gain unauthorized access, disrupting the service, scraping content at scale, or submitting false information or another person's details without permission.
