This Privacy Policy explains how Power Health Colorado ("we", "us") collects, uses and protects information submitted through this website. It applies to this website only. Care you receive as a patient is governed by our Notice of Privacy Practices under HIPAA.

Information We Collect

Through our appointment request form we collect the name, phone number, email address, preferred clinic location, new-or-returning patient status, any optional message you provide, and the page the request was submitted from. Please do not include medical details in the message field. We also collect limited technical information automatically, such as IP address, browser type, pages viewed and referring site.

How We Use Your Information

We use your information to contact you about scheduling, to answer your questions, to verify insurance benefits and to improve this website. Submissions are transmitted over HTTPS to our patient communication platform, which serves as our system of record. We do not store website form submissions in a separate database, and we do not sell or rent your information.

SMS and Text Messaging

By checking the SMS consent box on our form, you agree to receive text messages from Power Health Colorado. Message types include appointment reminders, scheduling and care notifications, and marketing messages. Message frequency varies. Message and data rates may apply. Reply STOP at any time to opt out, or HELP for help. Consent is not a condition of purchase or of receiving care.

SMS consent and phone numbers are never shared with or sold to third parties for marketing purposes. Numbers are shared only with the messaging provider that delivers messages on our behalf.

Protected Health Information (HIPAA)

Information you provide as an established patient during the course of treatment is protected health information governed by HIPAA and by our Notice of Privacy Practices, which is provided at your first visit and available at any clinic on request. This website form is intended for scheduling requests only and is not a secure channel for clinical information.

Cookies and Analytics

We use cookies and similar technologies for site functionality and analytics, and, where you consent, for advertising measurement. Non-essential cookies load only after you accept them in our cookie banner; you can decline at any time and continue using the site. Analytics events never include the values you type into our form. You can also block or delete cookies in your browser settings.

Sharing of Information

We share information only with service providers who operate this website, our patient communication platform and our analytics tools, and only as needed to provide those services; with insurers, adjusters or attorneys when you ask us to coordinate a claim; and where required by law.

Data Retention and Security

We retain website inquiry information only as long as needed for the purpose it was collected or as required by law, and we use administrative, technical and physical safeguards to protect it. No method of transmission over the internet is completely secure.

Your Colorado Privacy Rights

Under the Colorado Privacy Act, Colorado residents may request access to the personal data we hold, correction of inaccurate data, deletion of their data, a portable copy of their data, and may opt out of targeted advertising, sale of personal data or certain profiling. Data covered by HIPAA is exempt from these rights. To make a request, or to appeal a decision on a request, call us at the number below. We will verify your identity before responding.

Children's Privacy

This website is not directed to children under 13, and we do not knowingly collect their information online. Requests for a minor's care should be submitted by a parent or guardian.
